Register through OHIP office by calling to book an appointment * all applications must include three pieces of original documentation to prove Canadian citizenship, Ontario residency and personal identity * to replace a lost, damaged or stolen card visit ServiceOntario centre

All legally entitled residents of Ontario * must make a permanent and principal residence in Ontario and be present in Ontario for at least 153 days of any twelve month period * those below the age of 15 1/2 or over the age of 80 are eligible to mail-in for a health card renewal without having to visit a ServiceOntario centre
