| Eligibility - Population(s) Served: | 12 years and older * admitted patient or recently discharged from London Health Sciences Centre * willing to be screened and assessed * deemed medically stable * experiencing concerns related to substance use and/or withdrawal, gambling or gaming and would benefit from support to meet immediate needs upon discharge * must be facing barriers to connecting with addiction resources and supports on their own | ||
